Error 500 when submitting form

I set up the plugin as described, but when I try to submit the form nothing happens. Checkin in the browser’s console an error 500 appears. Here is the page: http://www.proclick.ro/andras-test/

I tried to reautentificate the plugin, changed the name of sheet (it contained spaces) but it’s still getting the same problem.
